Diagnostic Wax-up and Prosthetic Planning

Challenge

Patient suffers from severe dental phobia. Multistage reconstruction procedures are to be completed under general anesthesia at each step.

Goal

To conservatively reconstruct failed dentition, incorporating functional and esthetic changes to the existing teeth arrangement.

Approach

A team approach involving a surgical prosthodontist, anesthesiologist, and lab technicians. The desired esthetic appearance was developed first and then carried through to the final outcome.

Strategy

Diagnostic wax-up was transferred and tested with provisionals, followed by surgical and restorative phases in the fewest sessions possible.

Description

A comprehensive diagnostic workup was completed, including intraoral and extraoral photographs, intraoral scans, Cone Beam CT scans, examination, and facial scan. Using design software, we presented the patient with options for potential esthetic appearances, and the favorite was selected. This design was incorporated into the reconstruction plan, which required the removal of maxillary teeth, replacement with dental implants, and placement of ceramic crowns on the mandibular arch over existing teeth and some implants.
